Documents: Saddleback High School,
Santa Ana Unified School District

Thanks to Minh Thi Doan for these forms from Saddleback High School in Santa Ana Unified School District. She included an Academic Review Verification form (a simple checklist of the items reviewed during the individual conference - English and Spanish versions), a graduation flow chart for keeping track of credits, an Academic Success Contract (to be signed by the student and parent) for use with students needing extra support to improve academic performance, and a Weekly Progress Report (used in conjunction with the Academic Success Contract).

We also received several documents from Temple City Unified School District. Their forms include a Credit Evaluation, proficiency requirements, opportunities for addressing credit deficiencies, and college prep requirements. They also include a form to record "conduct, work habits, and effort."

Thank you to Megan Jara, a counselor at La Puente HS, for sending us these materials. They have developed an 1802 Conference Checklist, a Post High School Planning form, a 10th Grade Recovery Plan, an 1802 Parent Letter, an Academic Review form, and a Credit Recovery Options document. As we gather these from around the state, you will notice that many of them are beginning to look alike. Nice to see that people are avoiding reinventing the wheel. The objective is to work smarter, not work harder.

I just thought I would submit the forms that La Puente High School is currently using for our 10th grade 1802 conferences - although after seeing some others I plan to revise a bit for next year!  I love the sharing that is made possible through this website and hope that others utilize it as well.  Attached are a lot of different things I use; please feel free to post those you feel are pertinent to other counselors.

Documents: Moreno Valley Unified School District

Vanessa Gomez, a CASC Board representative and counselor in Moreno Valley Unified, has been gathering information to post on the Clearinghouse web site. To date, she has contributed several forms and documents developed within her district in response to the program requirements of AB 1802. In her introduction below, you get a good sense of her spirit as a counselor and as an advocate for her district. Thank you, Vanessa, for sharing these documents with colleagues around the state.

As AB1802 came to fruition, I spoke to several of my colleagues and found a wide variety of feelings related to the legislation, including hope, fear, and confusion. It was sad to hear that some districts did not give the counselors a voice in how AB 1802 was to be implemented and how funds were to be allocated. I am fortunate to work in a district where counselors had a voice. Moreno Valley Unified School district utilized a team approach, ensuring that counselors had a say in determining how the legislation would be implemented and how the funds would be utilized. Having a team work together to create a process and implementation plan helped the counselors reach goals that they had a say in.

MVUSD has had a strong history with counselor advocacy and the ASCA National Model. This district was one of the National Model school districts and has utilized data to drive programs. When counselors were given the task of addressing the requirements of AB1802, Valley View High School, like many schools in our district, linked AB 1802 to their intentional guidance goal. This site worked together as team to ensure that the identified students and parents were met with and found great success in doing so. At the end of this school year we will look at process, perception, and results data related to the implementation of AB1802. Attached are documents that show how Valley View High School implemented AB1802.

Putting the Pieces Together from Grantsburg Public Schools
 
We cannot recall how we came about this excellent resource. Nevertheless, one huge thank you goes out to the author(s) and counselors of Grantsburg Public Schools in Grantsburg, Wisconsin (http://www.gk12.net ). The 11 page guide addresses all you need to know regarding the 8 th Grade Individual Planning Conference. The document is well written, nicely presented, and includes such features as planning conference procedures and timeline, a conference invitation letter to parents, a News Release, conference protocol, and a Parent Feedback Form. The conference protocol includes some wonderful features such as Dreams for the Future, Learning Styles, and Multiple Intelligences Inventory. We are impressed with the quality of these materials and recommend that you adapt these to fit your districts circumstances. In addition, a PowerPoint is included that can be very helpful as a framework for presenting the value of the individual conferences as well as results of the sessions.
